(a)The applicant shall obtain a medical certificate from the chief medical officer or another authorized medical official of an appropriate jurisdiction which certifies that the applicant has the physical capacity to act in the capacity of an elector at the time of such application. The applicant shall also provide a statement of the applicant’s identity.
(b)An application for a replacement of an absentee ballot must be accompanied by proper identification as to the applicant’s name and date of birth, and the name and address of the address for which the application applies. The applicant shall indicate in the application whether the ballot is being returned in accordance with section 7 of chapter 110.
(c)An application for a replacement of an official vote-by-mail ballot must be accompanied by proper identification as to the applicant’s name and address, and the name and address of the address for which the application applies. The applicant, if applicable, shall also indicate in the application whether the ballot is being returned in accordance with section 11 of chapter 120.
(d)At least 30 days before the day on which an election is scheduled to be held, the applicant shall submit to the board of canvassers an affidavit stating that, at the time of mailing the ballot, there exists an adequate substitute ballot, as defined in section 12 of chapter 120.1.
(e)The board of canvassers, upon receiving an affidavit stating that there exists an affidavit that identifies the applicant as qualified, shall send a replacement absentee ballot by first class mail to the applicant, who shall return, sign and date the
